REGISTER OR MAKE AN APPOINTMENT

PATIENTS 

For medical assessments or treatments, you can either register with us directly or have your GP refer you to us for a specialist consultation.

Please bring the following with you to your appointment:

• Insurance card
• Medication plan
• Letter of referral from your GP, if applicable
• Any doctor's/hospital reports and X-ray findings, laboratory values, blood pressure values 
• Anticoagulant alert card, heart/pacemaker card

If you are unable to attend your appointment, please cancel at least 24 hours in advance.

DOCTOR'S REFERRALS FOR SPECIALIST CONSULTATION

We have prepared the following registration forms for our fellow medical professionals: 

• Cardiology – Angiology
• Phlebology – Leg/varicose vein treatment
• Ultrasound Examination (CEUS checklist)
• Dermatology

Please remember to enclose any documentation of previous examinations/hospitalisations.
