You are welcome to register for an examination or treatment either directly with us or
ask your primary care physician for a referral to a specialist.

If you are unable to attend, we kindly ask for early notification.
Please bring the following to your appointment:
- Insurance card
- Medication plan
- Any existing doctor/hospital reports and X-ray results, lab values, blood pressure readings
- Blood thinning medication card, heart/pacemaker card
- Completed patient form
